With over 30 years of service, South Jersey Against AIDS remains the only AIDS Service Organization in southeastern New Jersey; a region of 1,500 square miles with over a million residents comprised by Atlantic, Cape May, Cumberland and Camden counties (Including three state prisons housing over 7,900 inmates).

SJAA's challenge is to design and implement HIV care/treatment and prevention services that are based on the premise that all cultures are valued and that the strategies employed are culturally relevant.
SJAA defines culture broadly to include populations and subpopulations to include IDU, MSM, transgender, HIV+, women, and incarcerated.

Responsibilities:
? Conduct client intakes, both with inmates at South Woods State Prison and Bayside State Prison and in the community ? Develop care and/or risk reduction plans and discharge plans in conjunction with clients ? Assist clients in and with the acquisition of needed services through referrals to and facilitation of linkages with other departments, providers and/or organizations and agencies ? Provision of HIV Services Orientation to new inmates ? Provision of HIV education to the general correctional facility population ? Encouragement of members of the facility's identified populations with unknown status to be tested ? Provision of HIV case management services prior to release into the community (e.

, arranges for all levels of client care including medical, financial, mental health, housing, transportation and other social service needs.
) ? Provision of HIV case management to people living with HIV/AIDS in the community ? Accompany client to parole upon release ? Post-discharge follow-up services for a minimum of 3 months ? Input information into database ? Routinely solicit client feedback ? Assumes additional responsibilities as assigned III.

Experience:
Bilingual Community Care Coordinator will have a minimum of an associate or baccalaureate degree in human services from an accredited college or university.
Individuals without related academic degrees, relevant work experience conducted under the supervision of a human service professional can be substituted.
Bilingual Community Care Coordinator must have a valid New Jersey Driver's License and be able to obtain clearance from the Department of Corrections.
Bilingual Community Care Coordinator will possess strong interpersonal skills as well as excellent presentation skills.
A working knowledge of HIV/AIDS and related issues is helpful.

Travel:
During the first 3-6 months all new hires will be required to complete federal, state, and agency training.
This may result is colleagues traveling daily to additional sites.

Additional training may be needed beyond the first 6 months of employment.
Training is a condition of employment with SJAA.
Colleagues may also need to travel to attend SJAA meetings and/or functions such as our annual fundraiser.
